We encourage all members to maintain copies and records of their CERPS. In the event you find you have lost documentation of a certificate and need to receive a copy there will be a $20 charge. Click on the payment button below. Once payment is complete, the request form will open. Please ensure your popup blocker is turned off. Some work computers will block the form from opening. If you have completed the payment process and the form does not automatically open, you will need to send an email to flcassociation@gmail.com and request the form link. Payment will be verified and the form link sent.
